You must also click on, print out and sign the 'Baseball Ontario Consent Form' (below). A signed copy of the Baseball Ontario Consent Form is mandatory for a valid registration. Bring both forms with you to the registration location. The registration fee payment can be made by cheque or cash. We do not accept debit or credit cards.
